Policy:

3D Symposium 2008 has guaranteed the Doubletree Hotel to meet expected attendance requirements. In the event these guarantees are not met it is possible that the event may be canceled. If this occurs, be advised that 3D Symposium 2008 and the University of North Carolina Orthodontic Alumni Association will not be held responsible for registrants airfare, hotel or any other course related expenses that the participant may have arranged/incurred.

REGISTRATION WILL CLOSE MAY 31, 2008. YOU WILL NOT BE ABLE TO REGISTER BEYOND THAT DATE.

$275 each for each additional company representative.
(Two representatives per company is the maximum allowed)
